Site profile

Identify the adopting facility. These values populate the completed customization log and the document header.

State path

The state of operation determines how much state-law adaptation is required. Arizona sites inherit the HHC baseline unchanged; California sites require statute substitution and legal review.

AZ-Light path

Arizona

Same state as the HHC reference. State-law content carries over. You adapt site identity, personnel/vendors, and clinical differences only.

CA-Full path

California

Requires Arizona→California statute substitution and mandatory legal review. State-sensitive decisions are left open for your counsel.

Customization decisions

Each decision shows HHC's adopted answer. Keep it (Use HHC's answer) or enter your site's value. ⚑ must change marks site-identity and vendor items. CA marks decisions needing California legal review.
